Always Know Where You Are is a solo John song from Treasure Planet.  Stand Alone is a legit song, it was written by George for ABNG.  Pride is real, the Goos performed with Elmo on Sesame Street (still haven't watched it completely without cringing.  :D)

12D3 is new to me.   Doubt it's real.  Same for A Cry For Help and Somewhere over the Rainbow (unless they just covered part of it in concert or something).  I'll Be is Edwin McCain, there's all sorts of mislabelled mp3s floating around saying it's John.  It's not.

And yes, there is the 'x-rated' version of A Thousand Words.  Apparently you have to ask John to sing it that way, but he usually doesn't, because he sees kids in the audience.  The common version floating around is most likely from some show in 96 or 97.
